In a career as a Program Director of Case Management (also known as Director of Care Coordination Services), you will
oversee the delivery and documentation of program services.
You will also monitor program services to ensure compliance with regulatory requirements and contractual objectives.
Oversees and coordinates operations of case management / care coordination services.
Provides and reviews necessary communication and documentation for families, agencies, courts, and interested parties
to discuss family progress in meeting service plan goals.
Coordinates, guides, approves, and monitors family assessment and service plans; provides for regular review and
updating of service plans.
Supervises, administers, monitors, and evaluates established intervention procedures for the program and develops new
approaches for intervention as needed.
Provides direction for all case closure decisions.
Ensures appropriate level of direct contact with families per program requirements, and remains available to staff and
families 24/7, for crisis intervention as needed.
Assumes supervisory and administrative responsibility for care coordination services.
Promotes a culture of professionalism through role modeling professional boundaries, and respect.
Screens and hires direct program staff, and assumes responsibility for training staff. Provides and documents regular
supervision and accompanies staff in field as needed.
Administers financial planning, allocation, and disbursement of program budgets.
Coordinates referral process and ensures program capacity is maintained at appropriate levels; works with referral
sources and consumer groups to ensure appropriate referrals are made to the program.
Solicits feedback from consumers of the program activities including agency personnel and family members; develops
and installs effective data collection systems, provides appropriate data, supervises data gathering by others and
analyzes data from a variety of sources.
Establishes systems to predict and prevent potential problems and develops intervention strategies.
Oversees the fiduciary and compliance responsibility for case management services.
Implements and ensures compliance with all contractual, regulatory, program, and accrediting body standards.
Manages program occupancy, staff turnover, and other program variables that have a financial impact. Manages and
monitors staff changes that may affect job status or program compliance.
Reviews individual staff data to ensure that program guidelines are being followed, and used the data to improve
efficiencies and outcomes.
Monitors program budget to maintain revenues and expenses within budgetary guidelines.
Provides necessary documents and maintains certifications and licensures as required.
Builds and maintains relationships with children and families throughout services; builds and maintains relationships with
community funding entities, governmental agencies, service providers, and program consumers.
Masters degree in Human Services or a related field.
3 to 5 years of experience working with children and families including management and supervisory experience;
experience working with children and families in community-based programs or Boys Town Programs preferred.
Must possess a valid drivers license with a good driving record, plus the ability to provide own transportation (mileage
reimbursed) to complete travel requirements of the job. Must meet auto insurance requirements established by Boys
Town policy and/or State and Local laws.
This position is on-call (continuously or rotationally) to provide support.
Ability to lead a team of associates and to supervise family intervention services, be knowledgeable of, and comply with,
statutes and policies which affect target population.
Knowledge of regulatory requirements pertaining to youth care and the ability to learn, apply, and explain complex
regulations and policies governing child welfare processes and services.
Knowledgeable of community resources and service providers available in the geographical area.
Knowledge of strategic planning, resource allocation, leadership technique, and coordination of people and resources in a
Ability to communicate with individuals at all levels in the organization and with external business contacts in an
articulate, professional manner while maintaining necessary degree of confidentiality.
Computer skills in Microsoft Office.
Knowledge of the Boys Town Model.
