Summary
The Telephonic Care Coordinator is responsible for assisting the RN Case Manager with the Care Coordination needs of the
patient, ensuring communication and continuity across the healthcare spectrum. The TCC will assist with: Care plan
follow-up and patient/caregiver phone calls as directed by the RN Case Manager; Chart scrubs to ensure the patient’s
chart is up to date and assistance in identifying any gaps in care and quality; Scheduling appointments and
communicating with the PCP/staff as needed. The TCC will assist the RN Case Manager in providing for the comprehensive
healthcare needs of the patient to ensure quality, safety, and patient satisfaction.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Assist the RN Case Manager as directed in carrying out the patient’s individualized plan of care.
Perform monthly patient chart scrubs identifying care gaps, care plan follow-up needs, and any other care coordination
opportunities.
Make outreach calls to patients/caregivers as indicated.
Obtain medical records from specialists, hospitals, skilled nursing facilities, or other providers as indicated and upload
information in patient’s electronic medical record.
Assist RN Care Manager is providing information to patient/caregiver on disease state management, prescription drug
assistance programs, community resources, support groups, etc. as indicated.
Track time spent for Care Coordination (assessment, care plan, phone calls, medication refills and questions, scheduling
appointments, obtaining medical records/reports, etc.) by provider, staff, and care team for those patients enrolled in
Chronic Care Management and submit bill for services provided when indicated.
Effectively utilize the Electronic Health Record (Athena and Registry) to document as needed.
Other assignments as delegated.

Education and/or Experience - Bachelor’s degree from four-year college or university; or one to two years related
experience and/or training; or equivalent combination of education and experience.
Language Skills - Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the
most sensitive inquiries or complaints.
Mathematical Skills - Ability to apply basic mathematical concepts.
Reasoning Ability - Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret
an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and
concrete variables.
Computer Skills - To perform this job successfully, an individual should have knowledge of Electronic Medical Records
(Athena Database software); Microsoft Spreadsheet software and Microsoft Word Processing software; including Excel.
